NHS Scotland intends to establish a national Framework Agreement for the provision of a Digital Diabetes Prevention and Primary Hypertension Programme. The programme will support individuals identified as being at risk of developing Type 2 Diabetes and/or those diagnosed with primary hypertension, using digital interventions to improve health outcomes.
