Description
NHS Bristol, North Somerset and South Gloucestershire Integrated Care Board (BNSSG ICB) and NHS Gloucestershire Integrated Care Board are currently working together in a formal “clustering” arrangement to prepare for their merger into a single new organisation (name to be confirmed) effective from 1st April 2027.
BNSSG ICB (the commissioner) is seeking to secure a suitably qualified provider who has the capability and capacity to deliver a community-based pre-diabetes programme in South Bristol.
The service will provide a preventative intervention for adults aged 18-50 diagnosed with pre-diabetes, aiming to reduce progression to Type 2 diabetes, improve health outcomes, and address health inequalities. It will operate as a person-centred, community-based model integrated with primary care, complementing existing services such as the National Diabetes Prevention Programme (NDPP).
The service includes:
• 1:1 assessment within 6 weeks of diagnosis and a personalised support plan
• Education and self-management support to enable sustainable lifestyle change
• Follow-up at 6 months to review progress and outcomes
• Navigation to local services, acting as a link between primary care, community provision, and voluntary sector support
