The Borough Council of Calderdale (“the Council”) is commissioning a Community Support for Stroke Recovery and Prevention Service, which will focus on the prevention of, and recovery from, stroke.
The service will commence on 1st April 2027 for a period of five years, with the option to extend for a further two years. Key service components include:
- Early intervention and recovery support for stroke survivors
- Community-based rehabilitation and goal-setting
- Health promotion and stroke prevention programmes
- Carer support and family engagement
- Peer support groups and social networks
- Integration with primary care and hospital services
