The NHS Health Check programme (HC) is a 5-yearly contract with health care for adults aged 40-74 without a diagnosis of heart disease, kidney disease, stroke, or type 2 diabetes to manage Cardiovascular Disease Risk Factors (CVDRFs) for those conditions. A Health Check consists of an assessment of risk factors.
